How do you keep track of what changes you have made to your Forum?

How do you keep track of what changes you have made to your Forum?
Eg if you had to re-construct it, how would you know what changes you made
I started keeping a list of the changes in a word document. But I forgot to keep it up to date.

I’ve made minimal changes and those are usually the required changes needed for a fully functional forum. I don’t really keep track because I remember the changes since there aren’t many.
